Direct Procurement of Thoracic Organs Along with Abdominal Normothermic Regional Perfusion in Donation After Circulatory Death

چکیده
PurposeDirect procurement of thoracic organs as compared to using abdominal normothermic regional perfusion (aNRP) is the preferred method for organ in donors after circulatory death (DCD). The use aNRP improves outcomes from liver transplantation. We have developed a technique isolation during that allows successful co-procurement and organs.MethodsIn order achieve both brain volume loss must be prevented. After certification standoff period thoraco-abdominal incision performed. Blood collected donor right atrium or side-arm circuit prime care system (OCS) before vascular structures systematic way. 1. left pleura opened, lung retracted allow identification clamping descending aorta above diaphragm. 2. ascending clamped, aortic arch vented cranial clamp. Abdominal NRP can then commence. 3. inferior vena-cava clamped within pericardium. 4. superior azygos vein are tied off. 5. heart on side induction cardioplegia. diagram illustrates complete cavity enabling explantation delivery selective antegrade pneumoplegia while continue perfused 2-hours start procurement.ResultsBetween 2019-21 we successfully performed seven such procurements. Three lungs, three lungs one alone. All were implanted with immediate outcome all recipients.ConclusionDirect feasible option aNRP. This will potentially expand pool improve Direct organs. Regional perfusion by extracorporeal membrane oxygenation of abdominal organs from donors after circulatory death: a systematic review.
Organs from donors after circulatory death (DCDs) are particularly susceptible to the effects of warm ischemia injury. Regional perfusion (RP) by extracorporeal membrane oxygenation (ECMO) is increasingly being advocated as a useful remedy to the effects of ischemia/reperfusion injury, and it has been reported to enable the transplantation of organs from donors previously deemed unsuitable. The...
متن کاملThoracic aortic cannulation with antegrade perfusion for the procurement of abdominal organs.
The definitive acceptance of an organ as valid for transplant depends on the surgical team performing the multiorgan recovery; and unexpected difficulties can occur. The demographic characteristics of donors has changed, and some accepted donors can present difficulties in surgical technique and risky decisions on the validity of the retrieved organ or organs. An alternative method to the cannu...

OBJECTIVES A severe donor organ shortage leads to the death of a substantial number of patients who are listed for transplantation. The use of hearts from donors after circulatory death could significantly expand the donor organ pool, but due to concerns about their viability, these are currently not used for transplantation. We propose short-term ex vivo normothermic machine perfusion (MP) to ...

BACKGROUND The use of donation-after-circulatory-death (DCD) donors for lung transplantation has come into practice. In this study we investigated whether DCD lungs can be resuscitated after warm ischemia with normothermic ex vivo lung perfusion (EVLP). METHODS Four hours after cardiac arrest, beagle dogs were divided into two groups (n = 6 each): those with static cold storage (SCS group) an...
متن کاملDonation after circulatory death: current status.
PURPOSE OF REVIEW Donor shortage has forced transplant teams to explore new methods to increase the potential donor pool. Donation after circulatory death (DCD) has opened new perspectives and could be a valuable option to expand the brain-dead donors. The purpose of this review is to provide an overview of current practice and to identify remaining questions related to ethical and medical issu...
